Discuss proposed property partnerships with qualified substance-use recovery, rehabilitation or stabilization operators. Tell us the proposed program, responsible operator and required approvals.

Program administration partnerships

Request contract or program-administration support for a proposed residential initiative. Identify the independent operator and any required license or approvals; Revival does not provide currently licensed residential care.

Meetings and community programs

Explore prospective space for professional meetings, training and nonclinical peer sessions, subject to lawful use, occupancy and scheduling review.

From proposal to an approved program

These are partnership proposals, not operating care programs, guaranteed placements or confirmed room rentals. Before a program begins, the parties identify the property owner and responsible operator and confirm applicable licensing, land-use permission, building and fire requirements, and any required county contract or referral arrangement.

Revival Senior Living LLC provides contract and program administration. Mountain View Care Home LLC owns the Rocklin property. AMG Homes LLC is the property and hospitality operating partner for the scope assigned in the applicable agreement. Mission Ready Contracting LLC performs the coordination or contracting role identified in its agreement. These roles do not authorize licensed care, and a proposed program is not an active service.
